Describe the challenge you want to address:: Many seniors on Medicaid do not realize that they may qualify for health homes. With a new law this year, undocumented seniors who have 2 or more chronic conditions and Medicaid can benefit from health homes
What is the solution to the challenge?: Develop a health homes senior screening project. Go to houses of worship, immigrant organizations, and other places to reach out and screen seniors for health homes
