How does your organization plan to bring this idea to life?

The Connection Resource Center will offer comprehensive information and referral services to families engaged in our afterschool programs and Beacon Community Centers located in Briarwood, Woodhaven, Jamaica, and Ozone Park through targeted outreach, referrals for families, and Family Support Services for those in need of case management. 

What motivated your organization to apply to implement this idea?

The Connection Resource Center aligns with our integrated service model, which addresses interconnected needs, supports democracy, and builds connections between neighbors and needed resources in their community. QCH’s Family Support Services has a strong history of holistic and strengths-based case management.

How do you think this project will make a difference in your community?

The Connection Resource Center will support families holistically, addressing their immediate needs and connecting them to long-term solutions. Family Support Coordinators will support households dealing with multiple pain points, including housing instability, food insecurity, and benefits needs.

List at least one thing you're excited about related to this project!

We are very excited for the opportunity to bring additional services to Woodhaven and Briarwood and further serve families in Queens neighborhoods that can often be neglected. We also look forward to a year of great outreach events in the community that will be engaging for families and staff of the Connection Resource Center-alike!

How can someone participate or stay up to date on the project?

People can learn about our project through outreach events and flyers that we hand out during community events. Information sessions will also be held for the community to learn more about our project. People can participate by filling out the survey forms we created to assess the needs of their community and neighborhoods. People can participate by contacting a Family Support Coordinator via the contact information on flyers handed out during community events. These flyers will also be provided to our afterschool programs in partner schools within the communities of Woodhaven and Briarwood.
