Delivery of Nutritious Meals for Homebound Seniors

The Campaign Against Hunger (TCAH) will leverage its Health 360 wellness program involving cooking workshops and nutrition education, its Fresh Vibes Mobile Farmer's Market, cyber pantry choice model, SuperPantry, mobile and satellite sites to eliminate barriers that homebound seniors face in accessing healthy and nutritious food. The program aims to combat chronic diet-related illnesses and diseases such as diabetes, hypertension, and obesity and improve senior health and quality of life. The Campaign Against Hunger will target and reach 650 seniors through the food access programs. We will achieve our goals by collaborating with ten senior centers in the Brooklyn catchment area to provide monthly Health 360 classes led by TCAH's Executive Chef and Trainer, Tracey Johnson. Homebound seniors can register and shop for food online and have it delivered to their homes directly by TCAH, and those with no or limited internet access can call us. Our benefits access staff will enroll them over the phone. TCAH's Fresh Vibes Mobile Farmer's Market trucks will be on-site at each senior center once monthly, during the winter month, when FMNP checks are not available (January to July), and seniors can access upwards of twenty-four varieties of locally grown, farm-fresh fruits and nutrient-dense vegetables.
